What Is Psoriasis?

Psoriasis is more than a skin condition — it's a chronic, immune-mediated disease that affects over 8 million Americans.

The Science

Psoriasis occurs when the immune system mistakenly attacks healthy skin cells, causing them to multiply up to 10 times faster than normal. This rapid turnover creates the raised, scaly patches (plaques) that characterize the condition.

It's a systemic autoimmune disease — meaning it can affect more than just your skin. Up to 30% of people with psoriasis develop psoriatic arthritis, and the condition is also linked to cardiovascular disease, depression, and metabolic syndrome.

Common Symptoms

Plaque psoriasis (most common): Raised, red patches covered with silvery-white scales, often on elbows, knees, scalp, and lower back.

Other types include: guttate (small dot-like spots), inverse (smooth red patches in skin folds), pustular (white pustules), and erythrodermic (widespread redness).

Beyond the skin: joint pain and stiffness, nail pitting or thickening, itching, burning, and significant impact on emotional wellbeing.

What Are Biologics & How Do They Help?

Biologics are a revolutionary class of medications that have transformed psoriasis treatment over the past two decades.

How They Work

Your immune system uses chemical "signals" to tell your skin to grow new cells. In psoriasis, those signals get stuck in overdrive — telling your skin to grow up to 10× faster than normal. Biologics are precision-engineered medicines that quietly turn down the specific signals causing your psoriasis, without shutting down your whole immune system the way older drugs did.

For many patients, biologics achieve significant skin clearance — a level of improvement that wasn't possible with older treatments. They're typically self-administered injections taken every few weeks to every few months, depending on which one you're on.

Important safety information: Biologics work by targeting specific parts of the immune system, which can lower your body's ability to fight infections (including serious infections such as tuberculosis and fungal infections), and may rarely increase risk of certain cancers. Patients are screened for TB before starting and monitored throughout treatment. Common side effects include injection-site reactions, upper respiratory infections, headache, and rash. Your dermatologist will discuss all risks, benefits, and alternatives with you before prescribing any specific medication.

How Fast Do Biologics Work?

Most patients see improvement within weeks — not months. Here's roughly when you can expect to start seeing results:

First improvement
2–4 weeks
Noticeable clearing
6–8 weeks
Significant clearance
12 weeks
Peak results
16 weeks

In clinical trials, a substantial majority of patients on biologic therapy experienced meaningful skin clearance by week 16, with continued improvement through year 1. Speed and degree of response varies by medication class and individual patient.

⚠️ Results are not guaranteed. The timelines and outcomes shown above reflect average results from clinical trials and patient experience — they are not a promise of what any individual patient will experience. Response to biologic therapy varies widely based on disease severity, prior treatments, body weight, genetics, adherence to the dosing schedule, and other individual factors. Some patients see faster or more complete clearance than what's shown here; others see less improvement, see results more slowly, or may not respond at all and need to switch medications. Your dermatologist will discuss realistic expectations for your specific case before starting any treatment.
Important: Biologics require a thorough screening process including TB testing and review of your medical history. They are powerful medications with real benefits and real risks — your dermatologist will discuss both with you in detail before any prescription.

Other Effective Treatment Options

If biologics aren't the right fit — whether because of your psoriasis severity, your medical history, or simply because you don't want to inject yourself — there are excellent alternatives.

🧴 Topical Treatments

Creams, ointments, and foams applied directly to the skin. Often the first line of treatment for mild to moderate psoriasis, and effective for limited areas.

  • Prescription corticosteroids — anti-inflammatory creams that quickly calm flares
  • Vitamin D analogs — slow the rapid skin-cell turnover that causes plaques
  • Non-steroid prescription options — safe for long-term use, even on the face and skin folds
  • Vitamin A derivatives (retinoids) — help normalize skin growth

💊 Oral Medications (No Needles)

Take a pill instead of giving yourself a shot. Great for patients who don't want — or can't — inject themselves.

  • Newer targeted oral medications — once- or twice-daily pills with results approaching biologic therapy
  • Established oral immunosuppressants — used for decades, often weekly
  • Short-term oral options — fast-acting medications for acute flares
  • Oral retinoids — particularly useful for certain types of psoriasis

Frequently Asked Questions

Is the consultation really only $19?
Yes — for patients without insurance, or with an HMO plan who do not have an active PCP referral, we offer a discounted self-pay rate of $19 per consultation. Patients with PPO, Medicare, Medicaid, or an ACA plan will have their consultation billed to insurance. This $19 rate will not count toward your insurance deductible.
How does the consultation work?
After completing your online assessment, you'll be scheduled for a video consultation with a certified dermatology provider through our secure HIPAA-compliant video platform. During the call, your dermatologist will review your history, discuss your symptoms, and go over treatment options with you directly. You can also message your care team any time between visits.
How long until I see results from biologics?
Many patients notice improvement within 2–4 weeks of starting biologic therapy, with peak results typically around 16 weeks. Individual results vary based on disease severity, the specific medication, and other factors.
Are biologics safe for long-term use?
Biologics have been used for over 20 years with extensive long-term safety data. They can lower your ability to fight infections (including serious infections like TB) and may rarely increase risk of certain cancers, which is why we screen thoroughly before starting and monitor regularly. Your dermatologist will review all risks and benefits with you in detail.
Will I have to give myself injections?
Most biologics are self-administered injections (typically in the thigh or abdomen). Our care team provides video tutorials and one-on-one injection training, and many biologics use easy-to-use prefilled pens. Dosing frequency varies — some are every 2 weeks, others as infrequent as every 12 weeks. If you'd rather not inject yourself, oral medication options are also available.
What if I need an in-person visit?
If your case requires hands-on evaluation, biopsy, or specialized procedures, we'll refer you to one of our affiliated California Dermatology Institute locations. Your care is always our priority.
Do you treat psoriatic arthritis?
Yes. Several biologic medications are FDA-approved to treat both plaque psoriasis and psoriatic arthritis, so your dermatologist can often address both conditions with a single treatment. If your joint symptoms are severe, we'll also coordinate with a rheumatologist as part of your care plan.
